Reasons why treating pigmentation disorders is difficult

You need to know that pigmentation disorders that look similar to the naked eye are distinctly different.
From a skin histological perspective, epidermal pigments such as lentigines (blemishes, age spots, etc.) and freckles are concentrated in the basal layer, the lowest layer of the epidermis. On the other hand, dermal pigments such as melasma and nevus of Ota are located in the deeper dermis.
Furthermore, even pigments in the same location can respond significantly to treatment, requiring highly specialized knowledge to diagnose pigment lesions. Safe treatment can only be achieved by administering the correct treatment based on the location of the lesion and the characteristics of the melanocytes.
Distribution locations of major pigment disorders
Epidermal pigmentation: freckles, blemishes, solar lentigo, seborrheic keratosis (age spots), etc.
Dermal pigmentation: Melasma, ABNOM (Acquired Bilateral Nevus of Ota), Post-inflammatory hyperpigmentation, etc.

Precise analysis of the location and treatment prevent side effects and recurrence.

Consultation with a dermatologist x Identification of skin problems from multiple perspectives using the MarkView skin analyzer
Using continuous light source LEDs for more objective and accurate skin measurements
With four light sources, all implemented using LEDs, the effects before and after the procedure can be verified through images showing evenly distributed light.
By using a total of four light sources—UV light, polarized light, normal light, and gloss light—it is possible to integrate data captured from these four sources to accurately analyze various skin problems objectively.
In skin treatment, the most important thing is not to blindly remove visible pigmentation, but to understand your skin's constitution, the habits that have been negatively affecting your skin, and what the proper management plan is.
This is because even if the treatment suited to you is performed first, if post-procedure care tailored to your skin type is not carried out, your skin health may eventually deteriorate until the next treatment cycle, potentially leading to problems.
At PHD Dermatology, we provide detailed explanations of skin examination results using the MarkView skin diagnostic system. Furthermore, our board-certified dermatologist, a graduate of Seoul National University with a Ph.D. in Medicine, selects the appropriate lasers based on the diagnosis of various pigmented lesions to implement customized combination treatments. We also guide you through optimal care plans tailored to your changing skin.

It seems to be getting darker as time goes by, and I want to get rid of it as soon as possible.

The harder you shoot hoping for immediate big results, the more severe the skin irritation may become.
At PHD Dermatology, we do not proceed with unconditionally strong treatments that could actually be harmful.
Unless the pigmentation is very shallow, it is difficult to treat in a single session. Furthermore, attempting to remove a significant number of common facial pigmented lesions, such as freckles, lentigo, melasma, age spots, and nevus of Ota, all at once can cause excessive irritation to the skin, potentially leading to side effects such as blisters, burns, rashes, and skin depressions.
Appropriate laser stimulation
Pigment destruction, induction of collagen formation
New skin regeneration
Normal pigmentation treatment
Excessive laser stimulation
skin damage
Side effects such as indentation and scarring may occur
Treatment of excessive pigmentation
Then, how should it be treated effectively?
Combination therapy involving laser intensity adjustment tailored to the pigment depth and various treatments is required.
While the location and pattern of pigmentation are important, the most critical factor is its depth. Customized treatment is performed by selecting the laser and adjusting the output value based on the depth of the pigment.
01
Epidermal pigmentation that heals by forming a scab
Generally, for shallow pigmentation, treatment involves directly burning the area to create a large scab, or using a laser that reacts only to the pigment to create a small scab.
02
Dermal pigmentation treatment without scab formation
The deeper the pigment is located, the greater the risk of side effects if energy strong enough to form a scab is applied. Treatment focuses on inhibiting melanin activity and removing the pigment through repeated, step-by-step procedures.
03
Complex pigmentation treated by combining various devices
Insisting on only a single laser wavelength does not easily eliminate the problem. We efficiently treat mixed pigmentation disorders by evenly combining lasers tailored to the individual's condition and pigment characteristics.
Generally, epidermal pigmentation is removed by forming a scab, while dermal pigmentation is removed through repeated, step-by-step treatments without scab formation. However, since various types of pigmentation are usually mixed, laser treatment is often performed in combination.

Moving away from standardized treatment, we provide differentiated treatments tailored to the depth and type of pigmentation.

PHD Signature All-in-One, which performs treatments by combining various equipment and lasers
Just as individual skin types differ, skin treatments must also vary accordingly. PHD Dermatology provides personalized 1:1 care rather than integrated treatment, combining various pigmentation treatment devices according to the specific situation for combination therapy.

There are various types of pigmentation; can they all be treated with a single device?

The treatment method must vary depending on the type of pigmentation. Select from a variety of premium lasers to suit your skin problem.

Use of wavelengths with high absorption rates for pigmented lesions, GentleMax

It refers to a third-generation laser treatment method that uses laser equipment from Candela, USA, to make the skin clear and clean by improving blemishes, freckles, skin tone, elasticity, and blood vessels, and
Unlike IPL, this laser treatment uses wavelengths with a high absorption rate for pigmented lesions, making it not only excellent for pigmented conditions but also allowing you to simultaneously experience multi-dimensional effects such as increased elasticity, pore tightening, and wrinkle improvement.
Age spots, blemishes, freckles
The GentleMax procedure is sometimes called the "killer of age spots" because it effectively treats age spots, which are a common enemy of middle-aged and older adults. Additionally, because the laser reaches the dermis layer of the skin, it is effective for treating recurring blemishes and freckles.
Melasma, pigmentation
GentleMax's 3G Toning utilizes a quasi-long pulse Alexandrite laser beam to effectively remove melanin pigment deep within the skin, making it effective for treating pigmented lesions such as melasma and hyperpigmentation.
elasticity
The GentleMax 1064nm laser is a wavelength laser proven to enhance elasticity through the Genesis Lifting Technique, allowing you to simultaneously expect three-dimensional effects such as increased elasticity, pore reduction, and wrinkle improvement.
A New Proposal for Intractable Pigment Disorders

Repot laser with VSLS technology

Using a handpiece equipped with VSLS technology, which is the first to recognize lesions on its own, this laser device automatically adjusts the laser intensity in response to uneven pigment distribution, allowing for uniform treatment up to the depth of the lesion regardless of the amount of pigment, while rapidly inducing a wound healing response, and minimizes pain with a powerful contact cooling system.

Picolo Premium, a pico laser excellent for treating pigmented lesions

It is a picosecond laser that irradiates in units of one trillionth of a second, providing two wavelengths of 1064nm and 532nm with pulse widths of 450ps and 380ps, respectively, and enables rapid and fast treatment by breaking down pigment particles that cause photomechanical effects through microwaves that are 1,000 times shorter than nanosecond lasers.
General pigment laser
Various large pigment particles break down into small particles
Pico laser
Various coarse pigment particles are broken down into very small particles; that is, compared to general laser procedures, they are broken into smaller and finer particles, maximizing the treatment effect.
Uses a 694nm wavelength with high absorption rate by melanin pigment.

The Ruby Q Laser is excellent for treating pigmented lesions

Using 694nm light, which has a melanin absorption rate more than 10 times higher than existing pigment lasers, it can concentrate energy only on surrounding blood vessels and pigments, effectively removing almost all skin pigments such as blemishes, tattoos, age spots, nevus of Ota, and melasma. Additionally, by utilizing hexagonal beam technology to prevent energy overlap, it enables more stable treatment compared to circular beam devices.
Distribution of skin pigmented lesions
Selective destruction of only pigmented lesions deep within the skin
Enhanced treatment effect through melanocyte breakdown

The core of PHD Dermatology's pigmentation treatment: caring for even the unseen depths!

Along with laser treatment, our skincare program cares for even the latent melanin pigments deep within the skin.

As skin regenerative capacity and resistance decrease, skin tissue becomes damaged, and as recovery slows down, pigmentation occurs continuously.

At PHD Dermatology, combined laser treatment removes deposited pigments while inhibiting melanin-inducing cells to promote skin regeneration.

Environ Ionzyme
Environ's patented Ionzyme is a medical skincare product specialized in whitening, regeneration, and moisturization that utilizes sonophoresis (22KHz) and pulsed iontophoresis to rapidly penetrate ionized vitamins.
Cryo
Using cryo-electrotherapy, it quickly and effectively soothes skin problems such as sensitized skin, redness, and allergies, inducing skin condition stabilization and helping active ingredients applied to the skin penetrate deep into the skin.

Meticulous care, highly accurate diagnosis, thorough follow-up care
Our medical team, with over 20 years of experience and know-how, understands the skin better than anyone else and handles lasers expertly to provide premium medical services in a private space.
Confidence in your skin! PHD is a medical team of medical doctors who hold a Doctor of Medicine degree among dermatologists.
At PHD Dermatology, we provide customized treatments tailored to each individual's skin, ranging from diagnosis to laser procedures, through dedicated care by medical doctors from Seoul National University and board-certified dermatologists from Seoul National University Hospital. You can receive treatment not only for the removal of melasma and blemishes but also for stubborn pigmentation issues that are difficult to treat.
Expect a synergistic effect through the cross-treatment of various laser devices tailored to the characteristics of each pigmentation disorder, rather than a single treatment.
Since pigmentation disorders that appear similar to the naked eye differ not only in shape but also in the depth of the skin layers and the extent of their distribution, it is advisable to employ a combination of treatments tailored to individual skin characteristics rather than relying on a single method. At PHD Dermatology, we identify the skin types of Asians, who are prone to recurrence, and combine appropriate treatment and procedural methods.
Does increasing the laser intensity improve the effect? It is necessary to proceed with appropriate treatment and procedures tailored to each individual's pigmentation type.
While treating with high laser intensity may resolve the single concern of pigmentation, it can ultimately lead to secondary conditions as the skin barrier weakens. Therefore, pigmentation treatment must combine laser therapy that precisely targets and removes only the pigment with therapies that protect the skin barrier. At PHD Dermatology, we strive to go a step further than simple pigmentation treatment to minimize the likelihood of lesions recurring.

Precautions Before and After Pigmentation Treatment

We provide guidance on precautions to observe before and after the procedure for effective treatment and satisfactory results.
Wait for the scab that forms after the procedure to fall off naturally, and avoid rubbing it excessively.
You may apply makeup, wash your face, and take a light shower after the procedure, but please refrain from drinking alcohol, using saunas, steam rooms, or taking hot baths for one week.
Dryness or flaking is a normal reaction to heat and can be prevented with moisturizing creams, etc.
The lesion may appear darker for 2 to 3 days and gradually fade.
Redness and peeling may occur after the procedure, and the severity may vary depending on the individual.
